Free School and Sport Physical At Clay County Medical Center
Clay Co., Kan. — Free physicals will be offered to our youth from June 1 to September 30 this year! These physicals are for students who need sports physicals and school entry physicals.
You do not have to be a patient of their practice for a youth physical, and there is no follow-up obligation. Physicals will be offered every weekday throughout summer at any of our three convenient locations in Clay Center, Clyde, and Linn.
Your child’s health is a priority of CCMC, that’s why they have made a commitment to offer thorough and complete physicals for your growing students. This physical will be treated as a well-child visit, which is encouraged and covered by most insurance plans.
If this is not a covered benefit by your insurance plan, or you do not have insurance, you will not be charged for the physical. Concussion screenings will not be performed during the visit.
Prior to your appointment, visit www.ccmcks.org/familyphysicianspatientforms.php and complete the Sport Physical Form (PDF).
